I have a few suggestions for additional tools, or at least information to be included in the CCD, but I'm not sure how best to list them because they don't necessarily represent published works. They include - documentation of immunization records, vital signs and advance directives.
Also, in Michigan at least, there are tools required for documenting/reporting an indiviudals mental illness/developmental disability status - these are the DCH 3877 and 3878. I know that these are Michigan specific, but not sure if other states have a similar requirement.

The (Physician Orders for Life-Sustaining Treatment) POLST is a standard order form for advance directives & has been adopted by several states. It's not an assessment form, per se, but does contain standard information. www.ohsu.edu/polst Also, the next version of the MDS will have questions regarding immunizations and future versions of OASIS are also likely to include those questions. New versions of MDS and OASIS are currently in proposal/testing phases.

I added a fourth category under use cases called authorization of care. I guess we could put that under coordination of care but since payers don't take care of anybody I inserted an extra category.

I added a copy of a nursing home to hospital transfer form to the uploaded forms area of the wiki as an example of the type of information that our hospitals wish to receive. I did not, however, add it to the catalog of instruments because many of the areas will probably be covered by other actual tools.

I would bet that the Open Door Forum is related to the Research Triangle Institute project to create and test a PAC assessment instrument...the award was made in November. I have heard that the draft name of the instrument is the "PAC-PAI." I mentioned this work on one of our calls a few weeks ago.

The state of Ct utilizes a W-10 interagency referral form for hospital discharges as well as admission to SNF. There is also an MI/MR mandated for to determine appropriate SNF placement for psychiatriatric diseases. I can provide (mail) copies of those evaluation forms if needed.
